1. Subacromial Shoulder Impingement
Impingement occurs when the rotator cuff tendons get pinched between the acromion and the humeral head. This causes acute inflammation, resulting in a sharp pinching feeling in the shoulder whenever you lift your arm forward or sideways past shoulder height.
2. Rotator Cuff Tendinitis or Tendon Tears
Repetitive overhead lifting, gym workouts, or sudden strains can cause micro-tears and inflammation in the rotator cuff tendons (most commonly the supraspinatus). This leads to localized front shoulder pain and if the inflammation results in a tear, one might also experience shoulder weakness.
3. Biceps Tendonitis (Bicipital Groove Pain)
The long head of the biceps tendon runs directly down the front of the shoulder joint. Inflammation here causes a distinct throbbing or sharp pain in the front of the shoulder when lifting, rotating the arm, or carrying objects.
4. Shoulder Labral Tears (SLAP Lesions)
Damage to the cartilage ring (labrum) lining the shoulder socket can cause deep front-joint pain, instability, and a distinct catching or clicking sensation when reaching overhead.
5. AC Joint Arthritis or Degeneration
Wear and tear at the acromioclavicular (AC) joint—where your collarbone meets your shoulder blade—often causes localized pain on top and in front of the shoulder, worsening when reaching across the body or overhead.
Key Shoulder Impingement Symptoms to Look For
Shoulder impingement is one of the most frequent causes of sharp pain over shoulder. Recognizing its specific symptom pattern helps differentiate it from other joint issues:
Sharp pain lifting arm overhead: Especially when raising the arm sideways or forward past 90 degrees.
Pain reaching behind back: Discomfort when putting on a shirt, fastening a bra, or reaching into a car's back seat.
Nighttime shoulder pain: A dull ache that worsens at night or prevents sleeping on the affected side.
Loss of strength: Weakness when lifting, reaching, or placing items on high shelves.

Front shoulder pain when lifting arm forward: Strongly points toward biceps tendonitis or an anterior rotator cuff strain. Pain typically worsens when reaching straight out or performing overhead pressing movements.
Pain when lifting arm sideways (Arc of Pain): Classic sign of subacromial shoulder impingement involving the supraspinatus tendon. The sharpest pain usually occurs in the "painful arc"—between 60° and 120° of sideways elevation.
Everyday Activities That Trigger Front Shoulder Pain

Putting on a jacket or shirt: Requires combined overhead reaching and arm rotation, aggravating pinched tendons.
Reaching into the back seat: Extends the shoulder joint back under tension, straining the biceps tendon and front capsule.
Placing items on high shelves: Direct overhead load compresses the subacromial space, triggering sharp pain.
